Below are links to fill out forms/ sign releases. If you are unable to complete the forms this way, let me know and I can send you PDFs that you can print out, complete, and mail or email back to me.

How long is an appointment?
A typical session is 50 minutes. Occasionally, we may decide to meet for a session-and-a-half (75 minutes) or a double session (100 minutes).
What is your fee?
I charge $180 for a 50-minute session.
What if I need to cancel my appointment?
Cancellations made before 24 hours are not subject to a fee. Without 24 hours notice, barring an emergency, I will charge you your session fee for the missed appointment.
Can I use my insurance?
I am considered “an out-of-network” provider. Most insurance plans will reimburse for psychotherapy. I will provide you with a regular statement to submit to your insurance company for reimbursement.
Do you prescribe medication?
Psychologists do not have prescription privileges. Therefore, I am not able to prescribe medication. If, however, we should determine that medication would be helpful to you or your child, I will refer you to a psychiatrist who can evaluate medication needs and monitor medication while we continue to work together.
How long does therapy last?
Therapy can last anywhere from several weeks to several years, depending on the issue at hand and how responsive it is to therapy. Many people find they benefit from long-term therapy while others prefer shorter treatments.
